Literally (adding "by me", to help it sound more natural in Irish):

Gráite agam i gcónaí
Always Loved by me,
cuimhnithe agam i gcónaí,
always remembered by me,
i mo chroí i gcónaí
always in my heart

If space is a premium, I suggest switching to "forever", rather than "always", since it is shorter in Irish, but maybe leaving "always" in the last clause, because it seems to give a nice rhythm to the Irish version:

Gráite agam go deo [the word gráite has several meanings, but the word agam here helps to clarify it]
Forever loved by me,
cuimhnithe agam go deo,
forever remembered by me,
i gcónaí i mo chroí ["always" (i gcónaí) usually comes at the end, but here it helps with the rhythm if placed up front]
always in my heart.

Hmm, the word gráite is very uncommon as is the verb gráigh (to love).
Ní "ghránn" tú duine eile ach tá tú i ngrá leis nó tá grá agat air.
Irish often prefers nouns, and the noun here is an grá (the love).
You have love for s.o. or you are in love with s.o., but usually not "you love s.o.".

And furthermore: loved, remembered are passives and perfects. Both are uncommon categories in Irish.
Of course, Tá tú gráite agam (you have been loved by me or I have loved you) is correct grammar, but very unusual.


So, I'd use:
i gcónaí i ngrá leat = (you) always loved (lit. "always in love with you")
You could replace leat ("with you") by leis ("with him").

"cuimhnithe agam" is also unusual.

So, I'd say:
i gcónaí i gcuimhne ("always in memory")


All together (saving space with "i gcónaí" only once)
I gcónaí i ngrá leat, i gcuimhne, i mo chroí

But of course you can reiterate it:
