One bit of practical information you may not have considered is that that is an enormous amount of text for a tattoo. Your tattoo will have to be done in a much larger font than you may imagine, and there may not be room on your arm.

Type out the text in 36- to 40- point type and see how much space it would take. Or drop by a tattoo place with even the English version and ask if they think it could be done on your arm.

I've heard both bhí mé in ísle brí and bhí mé ar anchaoi before, and they also sound fine. If you use them, then you'd have either:
Shábháil tú mo shaol nuair a bhí mé in ísle brí
or:
Shábháil tú mo shaol nuair a bhí mé ar anchaoi
although what I offered is shorter, if space is a consideration.

So Ba suimúil ar chuala mé uaidh or Ba í an cheist ba suimúla acu would be correct (in those regions)?

That being said ba dhorcha would still be the C.O. form, right?
