I believe the translation is:
Chucu siúd a bhfuil grá agam dóibh a íobród

but want some additional opinions before I commit.

Also looking for confirmation on
mo chlann, mo ghrá

My family, my love

Thank you in advance for any help. Recently diagnosed with cancer and want to get this before chemo starts.

Yes. Ar son is much better, or mar gheall ar na daoine ...
Chun/chuig (beside do) is rather sacrifice to ... instead of for ...

Instead of a comma I'd write a relative particle: Ar son ... a íobród / a íobróidh mé

I forgot to respond to this part. The word clann can mean "family" in some contexts (it's where the English word clan comes from), but in modern Irish it is usually used to refer to one's children or descendants.

For "family" in a wider sense, there are basically two choices, muintir or teaghlach. The usage can vary by context, but teaghlach usually refers to the people in one's household, whereas muintir can refer to a one's extended family (and in context can also be used to refer to one's community or one's "people" or "tribe").

So, the choices would be:

Mo mhuintir, mo ghrá
My family, my love

and

Mo theaghlach, mo ghrá
My family, my love
